SECOND-STAGE COMBINED Asopa PLUS Mathieu' PENILE URETHROPLASTY
Introduction
We will show a new surgical technique for urethral and glans reconstruction, which uses the reconstructive concepts of Asopa and Mathieł.
Material and methods
Patient 49 years old who, after several surgical urethroplasties, he came to us with a first-stage Johanson urethroplasty.
He has already undergone several dilations.
Surgical technique:
Asopa phase: the scarred urethral plate is incised ventrally, to create an elliptical area where the dorsal-inlay buccal mucosa graft is placed and quilted to the glans.
Mathieł phase: the urethral plate is subsequently augmented ventrally by means of the preputial flap described by Mathieł.
Finally the new urethra is covered by the glans wings to complete glanduloplasty and the aesthetic reconstruction of the meatus.
Results
After 10 days when we remove the catheter we had a good result, without fistulae.
A normal-appearing glans and meatus.
Conclusion
The synthesis of the reconstructive surgical techniques described by Asopa and by Mathieł allows a good urethral reconstruction , and aesthetic reconstruction of the gland and the meatus, which are similar to the original aspect
At the same time this particular technique has a low risk of fistulae which are typical of this kind of surgery.
